Nomination Form Deadline: May 30 by 3:00PM 
 
Hall of Fame Criteria 
    • Nomination forms become property of the Prairie du Chien (PdC) Athletic Hall of Fame (HOF)
    • Nominations made to the PdC HOF must be made by an individual other then the nominee.
    • Nomination forms must be filled out by an individual who is acquanted with the nominee and his/her accomplishments.
    • Athletes must be a postgraduate of 15 years and honored with the distinction of Conference, Team, or Individual Awards.
    • In order for an individual to be considered for induction, they must have made a major contribution to the athletic program.
    • Nominees cannot be employed by district.
    • A coach must have coached for at least 5 years at any level.
    • Committee members will review nomination forms, pick top ten choice and vote for top four.
